If you live in New York City and were born in Puerto Rico, you can order your birth certificate.

You can apply for your own birth certificate if you are 18 years or older. To order birth certificate for a child under 18 years old, you must be the parent, legal guardian or heir, or authorized by the courts.

You can order a birth certificate with a credit or debit card through Puerto Rico's official government website or VitalChek.

Vital Chek

Each copy costs $13.40. If you want it delivered by express mail, the cost is $18.

The first copy is $5. Additional copies will cost $10 each.

If you live in Puerto Rico and are a veteran or 60 years old or older, you can apply for one free copy. Additional copies will cost $10 each.

You can order a birth certificate by completing and mailing an application to the Puerto Rico Vital Statistics Record Office.

Identification Requirements

You must include with your application a photocopy of a valid government-issued photo ID, such as a passport or driver's license. All other forms of government-issued photo ID will be subject to approval.

Cost

Each copy costs $5.

You must pay by money order payable to the Secretary of the Treasury of Puerto Rico.

If you ordered a new Puerto Rican birth certificate which contains incorrect information, mail it back to the Puerto Rico Vital Statistics Record Office along with the order receipt that you received with the certificate, indicating what needs to be corrected on the receipt.

If you have your original invalid birth certificate with the correct information, mail a copy of that with the order receipt and new birth certificate. There is no fee to get an amended copy.
