You can get birth certificate verifications for New York City and New York State Agencies, States other than New York and Federal Agencies, and for military personnel.

New York City and State agency personnel can request verifications electronically by sending an email to the Department of Health at nycdohvr@health.nyc.gov.

  • Enter "NYC verification" or "NYS verification" in the subject line.
  • Include the registrant's name, date of birth, mother's first and maiden name, and father's first and last name.
  • Submit the request from an official email address.

You should receive the verification within 5 business days.

You can get an electronic confirmation of birth certificate information, regardless of the place or date the certificate was issued. 

Authorized Federal and State agency users can search records throughout the country to verify birth certificate contents and request electronic certification.  The confirmation system is maintained by the National Association for Public Health Statistics and Information Systems (NAPHSIS).

Military personnel can electronically request and receive verifications for New York City birth certificates.

To request verifications, send an email to the Department of Health at nycdohvr@health.nyc.gov.

  • In the subject line, enter "Military Verification."
  • Include the registrant's name, date of birth, mother's first and maiden name, and father's first and last name.
  • Attach entitlement documentation, such as proof of application, induction, or enlistment documents.
  • Submit the request from an official military email address.

You will receive the verification within 3 business days.
